Based on some good references and my own experiences, here's critical items to ensure that your change initiative is succesful :
1. Change yourself first! - this is the most difficult things to do
2. Why you want to change? In what context ?
3. Be clear on what you want to change (what) and action steps (how)
4. Implication. Define what would be the implication to the current setting, to whom, etc
5. We must involve whoever will get the implication as earlier as possible, depent on the context and situation
6. Timeline : 3 months max.
7. Try to get small wins within 90 days
